Hospitalisation prior agreement request:
Hospitalisations can only be reimbursed if they have been previously approved by our Medical Department.
The application for prior approval must be completed by the doctor requesting hospitalisation and sent at least 15 days before the planned date of hospitalisation (or 3 days after admission in the event of a medically justified emergency) by post, fax or e-mail to:
