If your dog or cat has received medical care in Luxembourg, submit a claim for reimbursement in accordance with the terms of your insurance policy.

To make a claim for reimbursement under your dog and cat insurance policy: 1. Download the following Pet Insurance claim form:

2. Complete, date and sign this document

3. Have your veterinarian complete, date and sign it

4. Enclose the corresponding invoices and proof of payment for each act or intervention

5. And send in your form and supporting documents:
